Job summary

Willow Creek Health Care Center is seeking an experienced Licensed Vocational Nurse (LVN) to join as Admissions Nurse on a weekends-only schedule. The role focuses on welcoming new residents, coordinating admissions, and ensuring smooth transitions into skilled nursing care.

Qualifications

• Must hold an active California LVN license in good standing.
• CPR/BLS certification required.
• Experience with admissions, skilled nursing, or post-acute care preferred.
• Familiarity with electronic health records and medication reconciliation.

Responsibilities

• Perform nursing assessments for new admissions.
• Review hospital records, discharge summaries, and physician orders.
• Verify admission orders and enter them into the resident’s records.
• Complete medication reconciliation and communicate discrepancies.
• Coordinate with Admissions Dept and nursing leadership for upcoming admissions.
• Educate residents and families about the admission process and services.
• Maintain accurate documentation in the electronic health record.
• Follow HIPAA, infection prevention, and facility policies.
