Qualifications

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Follow-up and evaluation visits in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patients and report changes to clinical team.
• Collaborate with physicians and staff to deliver care.
