Job summary

Constellation Health Services seeks Per Diem Weekend LPNs to serve clients in the Norwalk, CT area. LPNs provide nursing care under RN supervision, following physician plans of care and ensuring safe, comfortable home-based patient outcomes.

Qualifications

• Graduate of an accredited practical nurse program.
• Licensed LPN in the state of employment.
• At least 1-2 years of LPN experience; home care preferred.
• Experience with catheters and wound care a plus.
• Reliable transportation to commute to patients' homes.

Responsibilities

• Provide nursing care per the patient's plan of care.
• Support end-of-life care and symptom management.
• Document services timely and accurately.
• Educate patient and family about diagnosis and plan of care.
• Coordinate with interdisciplinary team and report changes.
