Job summary

Hartford HealthCare at Home, Norwich, CT, seeks a Primary Care Nurse to provide hospice nursing in clients\' homes and community settings. This weekend position offers skilled care focused on outcome-based, high-quality services within our person-centered Care Model.

You will assess needs, develop and update care plans with the interdisciplinary team, coordinate services, and educate families to ensure compassionate, patient-centered care.

Qualifications

• Graduate of an Associate’s or diploma nursing program; BSN preferred.
• One year of nursing experience; home health or medical/surgical experience preferred.
• Current Connecticut nursing license.

Responsibilities

• Provides skilled nursing care to hospice clients in homes or community settings.
• Performs comprehensive nursing assessment based on referral data, interviews, and observations.
• Develops and updates a plan of care in collaboration with client and interdisciplinary team.
• Coordinates care with providers and documents plan of care and outcomes.
• Educates patients and families on care needs, signs, and symptoms; facilitates bereavement support when indicated.
