Oak Hill is seeking an LPN (Part-Time, every other weekend) in Wethersfield, CT. The role includes hands-on clinical nursing, health monitoring, and documentation to support individuals with diverse needs.

Must hold a CT LPN license, have CPR/First Aid, and possess strong assessment and communication skills. The position offers weekend hours, a competitive hourly rate, and a $4000 sign-on bonus in some cases.

Qualifications

• Current Connecticut LPN license in good standing.
• Graduate of an accredited Practical Nursing program.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Current CPR and First Aid certification, or ability to obtain upon hire.
• Strong clinical assessment, communication, and documentation skills.

Responsibilities

• Provide clinical nursing care and monitor health needs.
• Follow physician orders and nursing directives to ensure appropriate care.
• Administer medications and treatments as prescribed.
• Document nursing notes and medical records accurately and timely.
• Coordinate medical appointments and communicate with healthcare teams.
