Job summary

Qualifications

• Current LPN licensure in Connecticut.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and other treatments as needed.
• Develop and implement individualized care plans for residents, in collaboration with the healthcare team.
• Conduct regular assessments and monitoring of residents' condition, reporting any changes or concerns to the Charge Nurse or Physician.
• Maintain accurate and up-to-date records of patient care and treatment.
• Participate in inter-disciplinary team meetings and contribute to the development of care plans.
• Provide emotional and social support to residents and their families.
• Collaborate with other healthcare professionals to ensure seamless care delivery.
