Zusammenfassung

Trio Health Partners is seeking a Weekend In-House Nurse Practitioner to support internal medicine services in a Skilled Nursing Facility. You will build relationships with residents, families, staff, and providers while contributing to clinical leadership in a geriatric setting.

You will lead patient care initiatives, collaborate with the interdisciplinary team, and focus on proactive care to reduce hospitalizations in long-term care facilities.

Qualifikationen

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to maximize onsite time to develop robust caseloads
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

• Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner.
• Conducting new admission evaluations and monthly stability visits for long-term residents.
• Performing comprehensive physical examinations and evaluating changes in patient conditions.
• Prescribing medications and treatments as needed.
• Developing patient care policies and facilitating referrals to specialists.
• Coordinating advanced care planning and discharge visits.
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups.
• Supporting the physician-patient relationship and addressing gaps in care.
