Job summary

Trio Health Partners is seeking a Weekend In-House Nurse Practitioner to support the primary internal medicine provider at a Skilled Nursing Facility. This role focuses on building strong relationships with residents, families, facility staff, and the clinical leadership team in a geriatric care setting.

Responsibilities include admissions evaluations, monthly stability visits, comprehensive exams, prescribing treatments, and developing care policies while collaborating with physicians to ensure

Qualifications

• Active Illinois NP license (unencumbered).
• Master's degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with skilled nursing/long-term care exposure.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsibilities

• Ensuring continuity of care by communicating with the primary In-House NP
• Conducting new admission evaluations and monthly stability visits for long-term residents
• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care
