Trio Health Partners seeks a Weekend In-House Nurse Practitioner to support the primary internal medicine team in a Skilled Nursing Facility. You will conduct admissions, annual visits, and manage resident care plans, collaborating with physicians and staff to ensure high-quality geriatric care.

Qualifikationen

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Geriatric or adult medicine experience preferred.
• Strong clinical background in primary care and preventive medicine.
• Ability to engage elderly and chronically ill patients and families.
• Knowledge of Medicare, Medicaid and Health Plan benefits is a plus.

Aufgaben

• Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner.
• Conducting new admission evaluations and monthly stability visits for long-term residents.
• Performing comprehensive physical examinations and evaluating changes in patient conditions.
• Prescribing medications and treatments as needed.
• Developing patient care policies and facilitating referrals to specialists.
• Coordinating advanced care planning and discharge visits.
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups.
• Supporting the physician-patient relationship and addressing gaps in care.
