Job summary

Trio Health Partners seeks an experienced Nurse Practitioner to serve as the primary internal medicine provider within a Skilled Nursing Facility. You will build strong relationships with residents, families, and staff while contributing to clinical leadership and ongoing quality of care.

Qualifications

• Active Illinois NP license and NP credentials required.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience in SNF/long-term care preferred.

Responsibilities

• Conduct admission evaluations and monthly visits for long-term residents.
• Perform physical exams and evaluate changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop care policies and coordinate referrals to specialists.
• Coordinate advance care planning and discharge planning.
• Document assessments, plans, and follow-ups timely.
• Support physician-patient relationship and address gaps in care.
