Trio Health Partners is seeking a Weekend In-House Nurse Practitioner to support the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will build strong relationships with residents, families, staff, and providers while leading clinical care initiatives.

The role focuses on proactive geriatric care in long-term care settings, offering flexible weekend hours, with no nights or on-call, and access to an innovative two-way EMR system for efficient charting.

Qualifications

• Active Illinois Nurse Practitioner license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with focus on skilled nursing, long-term care, hospice, or palliative care.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to maximize onsite time to develop robust caseloads.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsabilités

• Ensure continuity of care by communicating with the primary In-House Nurse Practitioner.
• Conduct new admission evaluations and monthly stability visits for long-term residents.
• Perform comprehensive physical examinations and evaluate changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of patient assessments, treatment plans, and follow-ups.
• Support the physician-patient relationship and address gaps in care.
