KeystoneCare is hiring an RN/LPN Caregiver to provide skilled in-home nursing for a patient with a tracheostomy, ventilator, and PEG tube in Ambler/Blue Bell, PA. You will work closely with the care team to implement the treatment plan and ensure patient comfort and safety.

The role requires strong clinical judgment, compassionate patient-centered care, and the ability to work independently in a home setting.

Qualifications

• Graduate of an accredited Licensed Nursing program.
• Current Pennsylvania Nursing license in good standing.
• Ability to read, write, and communicate effectively in English.
• Experience with tracheostomy, ventilator, and PEG tube care preferred.
• One year of home health experience preferred.

Responsabilités

• Provide skilled in-home nursing care for a patient with a tracheostomy, ventilator, and PEG tube.
• Monitor and assess the patient's physical, emotional, and clinical condition; execute the care plan.
• Administer medications and treatments as prescribed; follow care instructions.
• Assist with activities of daily living, mobility, transfers, and device management (Hoyer Lift).
• Maintain a clean, safe home environment and document care using EVV system.
• Coordinate with RN Case Manager and accompany patient to appointments as needed.
