Job summary

Brighton/Suncrest Hospice is seeking a Social Service Weekend Coordinator to implement standards of care for medical social work services and ensure adherence to the service agreement. The role centers on coordinating PRN staff and visits, working with social workers, chaplains, and nursing leadership to address patient needs on weekends.

Qualifications

• LCSW preferred.
• MSW from CSWE-accredited program.
• Healthcare or hospice leadership experience preferred.
• Excellent verbal and written communication skills.
• Strong organizational skills; ability to prioritize and work independently.
• Strong EMR documentation, reporting and auditing skills.
• Must have a valid driver''s license and reliable vehicle.

Responsibilities

• Coordinate PRN staff and visits for weekends (Fri evening to Sun).
• Collaborate with social workers, chaplains and nursing leadership to address patient needs.
• Assess and prioritize patient and family needs; schedule PRN and part-time staff.
• Triage urgent psychosocial issues with weekend team.
• Communicate with patients, families, senior living facilities and community agencies.
• Complete charting in the EHR promptly and maintain patient records.
