Job summary
Community Hospice and Palliative Care in Florida seeks an experienced RN Baylor to provide skilled nursing care for hospice patients on weekends and one weekday, coordinating symptom management, education, and end-of-life planning.

Qualifications
- Two years of experience in acute care, oncology, home health, or hospice preferred.
- Florida RN license and CPR certification required.
- Valid Florida driver's license and reliable transportation.
Responsibilities
- Perform skilled nursing assessments and interventions per care plans.
- Educate patients and caregivers on disease progression and end-of-life expectations.
- Collaborate with patients, families, and practitioners to manage symptoms.
- Maintain EMR documentation in real time and accuracy.
- Participate in IDG meetings and provide end-of-shift reports.
- Uphold nursing standards and participate in ongoing education.
- Support agency quality assurance and comply with protocols.
