Twilight Hospice & Palliative Care is seeking a compassionate PRN Hospice Registered Nurse for every other weekend (Fri 4:30pm – Mon 8:00am) to provide skilled nursing, triage calls, and occasional visits in the Ft Worth/Dallas County area. You will work with a dedicated interdisciplinary team to ensure comfort, dignity, and high-quality end-of-life care.

Qualifications include a Texas RN license, hospice experience preferred, strong assessment skills, and the ability to work independently.

Qualifications

• Current Registered Nurse license in the state of Texas.
• Minimum of one year of hospice experience preferred.
• Strong clinical assessment and decision-making skills.
• Compassionate, patient-centered approach to care.
• Valid State Driver's License and reliable transportation.
• Ability to work independently and manage time effectively.

Responsabilités

• Provide skilled nursing care for hospice patients, ensuring adherence to the patient’s care plan.
• Conduct comprehensive patient assessments, including physical, emotional, and psychosocial needs.
• Coordinate care with the interdisciplinary team, patients, and families to ensure continuity and quality of care.
• Provide education and support to patients and families, promoting dignity and comfort.
• Administer medications and treatments as prescribed, following agency policies and Medicare guidelines.
• Document care provided and any changes in patient status promptly and accurately.
• Communicate effectively with patients, families, and the interdisciplinary team to address care needs.
