Qualifications

• Active Minnesota RN license required.
• Experience in hospice, ICU, ER, oncology, med-surg, home health, or palliative care valued.

Responsibilities

• Provide evening hospice nursing visits throughout the Twin Cities Metro.
• Assess patient needs, provide symptom management, medication administration, and comfort-focused care.
• Educate and support patients, caregivers, and family members.
• Deliver care in private homes, assisted living communities, memory care, and skilled nursing facilities.
• Collaborate with physicians and the interdisciplinary hospice team.
• Complete timely and accurate EMR documentation.
