Job summary

Superior Hospice in Minnesota is seeking a licensed RN to provide scheduled hospice visits in patients' homes and facilities. You will deliver symptom management, education, and compassionate care with an emphasis on end-of-life comfort.

Qualifications

• Hospice or end-of-life care experience preferred.
• Ability to build compassionate relationships with patients and families.
• Strong communication and interpersonal skills in a home and facility setting.

Responsibilities

• Provide scheduled hospice visits in patients' homes and facilities.
• Assess symptoms and provide comfort-focused nursing interventions.
• Educate patients and families throughout the hospice journey.
• Collaborate with the interdisciplinary team for individualized care.
• Complete timely and accurate EMR documentation.
