Job summary

AMEDISYS HOME HEALTH is seeking a Hospice Weekend On Call Baylor RN to respond to after-hours requests from our Triage RN. The role includes telephone responses or in-person visits with skilled assessment and interventions, potentially after-hours admissions and end-of-life visits.

Salary range is $84,000–$86,000 annually plus mileage. Schedule includes Sat 8a-8p, Sun 8a-8p, and Mon 8a-6pm with comprehensive benefits following hire.

Qualifications

• Current RN license applicable to assigned state(s).
• At least 1 year of nursing experience.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care in collaboration with the care team.
• Communicates patient progress to the clinical manager and physician.
• Maintains patient care progress records accurately and timely.
• Records pain and symptom management changes/outcomes.
• Educates families on patient condition and care.
• Makes referrals to other hospice disciplines as needed.
• Performs on-call responsibilities and services as assigned.
• Performs other duties as assigned.
