Job summary

Providence Hospice and Palliative Care is seeking a Hospice Registered Nurse (RN) for weekends only. This full-time role in St. John, IN requires in-person visits to patients at homes and facilities, with travel across Lake County.

Weekend shift runs Friday 4:00 PM to Sunday 8:00 AM, with a back-up nurse for Saturday/Sunday support. A strong team supports patient care with comprehensive benefits. The ideal candidate holds Indiana RN licensure, at least 1 year hospice/home health experience, a

Qualifications

Active RN license in Indiana.
At least 1 year RN experience in Hospice or Home Health.
Driver's license in good standing.
CPR certification preferred.

Responsibilities

Plans, coordinates and provides hospice care services for patients and caregivers.
Delivers treatments and diagnostic/palliative procedures under medical direction.
Documents actions and observations.
Reports to attending physician and IDT on patient status and treatment responses.
Coordinates follow-up care with appropriate disciplines.
Supports and assists caregivers in patient care.
Identifies hazards and ensures safe environment.
Completes documentation per policy.
Responds to changes in patient condition and communicates with team.
Participates in orientation and IDT meetings.
Complies with CDC, OSHA, Joint Commission, state and federal regulations.
Travels to patients' homes.
