Providence Hospice and Palliative Care, a division of Providence Life Services, is seeking a Weekend Hospice Registered Nurse (RN) for St. John, IN. This full-time, in-person role requires weekend visits Friday 4:00 PM to Sunday 8:00 AM, with a back-up nurse during weekends.

Travel to patient homes and facilities is expected, with a comprehensive benefits package and mileage reimbursement. Ideal candidates hold an active Indiana RN license with at least 1 year of hospice or home health

Formación

• Active Indiana RN license.
• Minimum 1 year RN experience in Hospice or Home Health.
• Valid driver's license and reliable transportation.
• CPR certification preferred.
• Must pass a physical, drug screen, background checks and reference checks.

Responsabilidades

• Carries out treatments and required diagnostic and/or palliative procedures under medical directions.
• Documents all actions and observations.
• Reports to attending physician and IDT physical and emotional conditions and reactions to treatment.
• Coordinates follow-up for patient care needs after visits with appropriate disciplines.
• Supports, teaches and assists caregivers in providing patient care.
• Identifies and reports hazards in the environment.
• Completes documentation per policy requirements.
• Respond to changes in patient condition.
• Participates in orientation, in-service and IDT meetings.
• Travel to patients' homes as required.

Description

Providence Hospice and Palliative Care is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice and Palliative Care is seeking a Hospice Registered Nurse (RN),Weekends Only, Full Time, in St. John, IN and throughout Lake County. The Weekend Hospice RN for Providence is an integral team member who continues our excellent patient care into the weekend. This position requires in-person visits as scheduled and as needed to patients in the community and at facilities. This position is ideal for nurses who value meaningful patient connections and a consistent weekend scheduled. Our Weekend nurse will have the support of an amazing team, who work towards the same goal.

Schedule Available: This is a full-time, exempt position in the northwest areas of Indiana and requires regular travel. The schedule for the position requires a weekend commitment of Friday, from 4:00PM to Sunday, 8:00AM. Scheduled visits will occur during the weekend as well as visits that are unplanned that require a nurse visit. A back-up nurse will be available for support on Saturday and Sunday, 8am to 4pm. This position comes with the full benefit’s package. Mileage reimbursement is $0.76/mile.

• Respond to reported patient change in condition.
• Participates in orientation, in-service and IDT meetings.
• Perform all functions in compliance with CDC, OSHA, Joint Commission, State and Federal regulatory bodies.
• Travel to patient’s homes.

Requirements

• Active Registered Nurse (RN) License in the state of Indiana.
• At least 1 year of RN experience in Hospice or Home Health setting.
• Drivers License in good standing.
• Reliable transportation.
• CPR certified, preferred.
• Must pass a physical, drug screen, 50-pound lift test, fingerprinting for criminal background check and complete a reference check.
