Hartford HealthCare at Home is seeking a weekend Primary Care Nurse to provide skilled nursing care to hospice clients in patients' homes or community settings within Connecticut. The role emphasizes end-of-life care, patient comfort, and family support, with focus on comprehensive assessments and care planning.

Candidate must hold an active CT nursing license and have at least one year of nursing experience, preferably in home care or hospice; BSN is preferred.

Qualifications

• Associate's degree or nursing diploma required; BSN preferred.
• Minimum one year of nursing experience; home care or hospice preferred.
• Connecticut license to practice required.

Responsabilités

• Provide skilled nursing care to hospice clients in home settings.
• Conduct comprehensive patient assessments and develop plans of care.
• Coordinate care with the interdisciplinary team and patient/family.
• Supervise LPNs and Home Care Aides in implementing plans of care.
• Document care delivery and patient outcomes in medical records.
