Job summary

Responsibilities

Provide skilled nursing care to hospice clients in homes or community settings.
Review referrals and initiate contact with client and family.
Conduct complete nursing assessments and develop care plans with the client and disciplines.
Coordinate care with physicians and the interdisciplinary team.
Direct and supervise Home Care Aides and LPNs.
Document timely reporting and maintain client records.
