Hartford HealthCare at Home is seeking a Primary Care Nurse to provide skilled nursing care to hospice clients in patients' homes, with a weekend schedule totaling about 32 hours. The role requires active CT nursing licensure and an ADN (BSN preferred). Experience in home care or medical/surgical settings is valued.

The position emphasizes interdisciplinary collaboration, patient education, and delivering high-quality, compassionate care to clients and families in their homes across Connecticut.

Qualifications

• Graduate of an Associate’s or diploma program in nursing (ADN) required.
• CT license to practice nursing required; BSN preferred.
• One year of nursing experience required; home care or medical/surgical experience preferred.

Responsabilités

• Provide skilled nursing care to hospice clients in home or community settings.
• Perform nursing assessments and develop care plans in collaboration with the client and the care team.
• Coordinate care with physicians and the interdisciplinary team to ensure high-quality, patient-centered care.
• Document care plans and patient progress in agency records; ensure timely reporting.
• Deliver patient and family teaching to support at-home care and symptom management.
