Moments Hospice is seeking a Weekend Registered Nurse to lead clinical care for a dedicated caseload, providing compassionate hospice services on Saturdays and Sundays. You will assess patients, manage symptoms, coordinate care, and support families through a meaningful stage of life.

Qualifikationen

• Active RN license in the state you work in.
• Proficiency with EMR systems.
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Aufgaben

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
