Job summary

Moments Hospice is seeking an RN Case Manager to lead clinical care for patients and families in a hospice setting. You will assess needs, coordinate plans, and deliver skilled nursing with symptom management. Enjoy leadership access, reasonable caseloads, and a path to grow within a supportive team.

This role includes a fleet car program and comprehensive benefits. The schedule is Monday–Friday, with a standard daytime shift, and a competitive salary range of $75,000 to $90,000 per year.

Qualifications

• Active RN license in the practicing state.
• Experience with end-of-life or hospice care is preferred.
• Excellent communication and compassionate approach.
• Ability to manage multiple patients with minimal supervision.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care with patients and families
• Provide skilled nursing care, including pain and symptom management, meds, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations
• Coordinate care across caseload and liaise between patients, families, physicians, and care team
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care
