Job summary

Badger Hospice, LLC is seeking a skilled Hospice Visit Nurse (RN) to provide compassionate, in-person care to hospice patients and families on weekends. You will deliver skilled nursing visits, respond to urgent patient needs, and collaborate with our interdisciplinary team to ensure comfort and continuity of care.

Schedule includes every Saturday & Sunday, 8:00AM – 5:00PM, with holiday coverage as needed. You will assess, document, and coordinate care while supporting patients and families

Qualifications

• Current Wisconsin RN license in good standing.
• Hospice, home health, med-surg, or critical care experience preferred.
• Strong assessment, communication, and clinical judgment skills.
• Valid driver's license, reliable insured vehicle, and ability to travel between Brookfield and Burlington service areas.

Responsibilities

• Provide skilled nursing visits, admissions, follow-up, and death visits as needed.
• Assess patient needs, manage symptoms, and coordinate care with the hospice team.
• Educate and support patients and families throughout the hospice journey.
• Maintain accurate documentation and communicate changes in patient condition.
