Job summary

Pathways Healthcare in Boston is seeking a dedicated Registered Nurse to join our home health and hospice team. You will provide compassionate, patient‑centered care in patients' homes, assess needs, implement plans of care, and collaborate with physicians and the interdisciplinary team to support recovery.

New nurses are welcome and receive a comprehensive training program; experienced RNs with geriatrics experience are encouraged to apply.

Qualifications

Licensed RN in MA with current license.
2+ years of nursing experience, geriatrics preferred.
New nurses are encouraged to apply with our training program.
Excellent bedside manners and communication.
Knowledge of healthcare laws and regulations.
BLS CPR certification required.
Valid drivers license and reliable vehicle.

Responsibilities

Initiates preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
Assesss patient’s needs through triaging and providing in-home care when needed.
Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
Responds to inquiries and concerns from patients, their caregivers, and their medical team.
Consults patients and caregivers before, during and after treatment.
