Job summary

Topkare Hospice INC is seeking a Weekend Hospice Registered Nurse (RN) to provide compassionate end-of-life care in patients’ homes and community settings. You will assess patients, manage symptoms, and support families with empathy during weekend shifts.

You will collaborate with interdisciplinary teams, document thoroughly in EHR systems, and respond to urgent needs while upholding HIPAA and hospice standards. On-call weekend coverage is required.

Qualifications

• Valid RN license in the state of practice.
• Proficiency in EMR/EHR systems including Hospice MD, HCHB and Netsmart.
• Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods in hospice care.
• Strong assessment skills and ability to manage acute pain and complex symptom control.
• On-call shifts required.
• Experience in home care or community-based nursing; Med-Surg or long-term care is beneficial.

Responsibilities

• Conduct comprehensive patient assessments, including vital signs, symptom management, and psychosocial evaluations.
• Administer medications and treatments such as IV infusions, pain management protocols, and wound care.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Provide education and emotional support to patients and families, guiding them through complex medical decisions.
• Maintain accurate medical documentation within electronic health records (EHR) systems.
• Respond promptly to urgent patient needs via triage protocols and coordinate with emergency services when necessary.
• Participate in ongoing training related to hospice & palliative care and advanced symptom management.
