Job summary

Residential Home Health and Hospice in Northbrook, IL is seeking a Weekend Registered Nurse (RN) to deliver compassionate hospice care in patients' homes. This role offers autonomy within an interdisciplinary team and a $9,000 first-year success bonus tied to milestone achievements.

Qualifications

• Active RN license in the state you work
• Two years of RN experience and hospice care preferred
• Valid driver's license and ability to travel to patient homes (approx. 20% of time)

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes
• Respond to urgent needs with assessments, symptom and pain management, and end-of-life support
• Support and guide patients and families on weekends, with crisis coverage as needed
• Coordinate care with the interdisciplinary team and maintain EMR documentation
• Travel to patient homes within assigned territory
