Job summary

Beth Israel Lahey Health is seeking a Weekend Baylor Hospice RN to provide compassionate home-based palliative care, applying the nursing process to assess, plan, implement and evaluate patient needs. Qualifications

• Active MA RN license.
• BLS certification required.
• Associate’s Degree or BSN preferred.
• 1 year direct care nursing experience required.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Provide skilled nursing care and prescribed treatments to patients and families in their home.
• Documentation of services using problem-oriented records.
• Plan for continuity of care with nursing team, patient, family, and other disciplines.
