Qualifications

• Experience in hospice care, home health, oncology, and/or pain management preferred.
• Must maintain a valid driver's license and good driving record.
• Equivalent education/experience may be acceptable.

Responsibilities

• Provide prescribed medical treatment and personal care services.
• Monitor and evaluate pain and symptoms.
• Serve as a liaison to the patient's physician(s)
• Participate in care conferences and quality improvement activities.
