Job summary

Superior Hospice in Plymouth, MN is seeking a Hospice RN Float to support Admissions, Case Management and Field Visits across our growing hospice program. The role offers clinical autonomy and strong teamwork, with on-call and weekend rotation as part of the schedule.

You will manage admissions, conduct evaluations, coordinate care, educate families, and respond to urgent clinical needs while maintaining high quality patient care.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred, or strong clinical background in ICU, oncology, palliative care, home health or complex care settings.
• Excellent assessment skills, critical thinking and independent clinical judgment.
• Comfortable managing changing priorities while maintaining exceptional patient care.
• Outstanding communication and relationship-building with patients, families and teams.
• Reliable transportation and willingness to travel throughout the service area.
• Compassionate nurse who enjoys learning, problem-solving and making a difference.

Responsibilities

• Complete hospice admissions, eligibility evaluations, informed consents, education and initial care planning.
• Provide case management support including symptom management, medication oversight and ongoing care coordination.
• Complete routine, PRN, urgent and crisis field visits throughout the Twin Cities metro area.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide interdisciplinary care.
• Educate patients and families on disease progression, comfort-focused care and hospice services.
• Maintain timely EMR documentation and ensure compliance with Medicare Conditions of Participation and hospice regulations.
• Adapt quickly to changing clinical priorities while delivering patient-centered care.
