Job summary

Hartford HealthCare at Home is seeking a Primary Care Nurse to provide skilled nursing care to hospice clients in patients’ homes and community settings. This weekend position offers 32 hours per week with compensation for 36 hours, and work at 227 Dunham St, Norwich, CT 10414.

Qualified candidates hold an Associate’s or diploma in nursing with CT licensure. One year of nursing experience or exposure in medical/surgical or home care is preferred; BSN is a plus.

Qualifications

Graduate of an Associate’s or diploma program in nursing.
CT license to practice required.
One year nursing experience or one year in a specialty.

Responsibilities

Provide skilled nursing care to hospice clients in their homes or community settings.
Review referrals and initiate contact with client and family.
Develop a plan of care in collaboration with client and disciplines.
Follow interdisciplinary team processes and coordinate care with providers.
Direct LPNs and supervise the plan of care.
