AccentCare, Inc. is seeking an After Hours Registered Nurse (RN) for Hospice, full-time, weekends in Mississippi. The role involves on-call shifts with scheduled visits, no remote work, and coverage for Hinds, Madison, Rankin counties and surrounding areas.

You will assess patients, coordinate with the care team, and provide education and support to patients and families. The position requires active RN licensure, relevant hospice/home health experience, and willingness to participate in on-call

Qualifications

• Current RN licensure in the applicable state; associate degree preferred.
• Willingness to participate in an on-call rotation and have reliable transportation.
• Strong communication and organizational skills; ability to collaborate with an interdisciplinary team.
• Proficiency in Microsoft Office Suite and EMR systems.

Responsabilités

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and the IDG to develop, implement, and evaluate care plans.
• Maintain timely and accurate documentation and adjust care plans as patient status changes.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations, admissions and care transitions.
• Coordinate care if serving as Care Manager, including scheduling, med management, and supervision of staff.
• Respond to triage calls, perform visits and assessments when on-call; participate in IDG meetings.
