Job summary

AccentCare is seeking an After Hours Registered Nurse (RN) for Hospice to join our full-time weekend team in Mississippi. Training occurs Monday through Friday for 6–8 weeks, followed by on-call shifts with scheduled visits Friday 4p to Monday 8a.

This role requires current RN licensure and strong communication skills to provide compassionate, patient-centered care. Ideal candidates have hospice or home health experience, be prepared for an on-call rotation, and demonstrate reliable

Qualifications

Active RN license in the applicable state.
Experience in hospice, home health, or related nursing preferred.
Willingness to participate in an on-call rotation and maintain reliable transportation.

Responsibilities

Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and IDG to develop, implement, and evaluate individualized plans of care.
Maintains timely and accurate documentation and communicates effectively with the care team to coordinate resources.
Provides education and emotional support to patients and caregivers and participates in on-call rotations, admissions, and care transitions.
If serving as Care Manager, coordinates all aspects of patient care including scheduling, medication management, and supervision of staff.
If serving in On-Call capacity, responds to triage calls, performs visits, and coordinates care with the Administrator On Call.
