Job summary

Family Home Health Network is seeking a dedicated Home Health Nurse to deliver compassionate, evidence-based care in a patient’s home. You will perform comprehensive assessments of patients, families, and environments to tailor a precise plan of care and ensure timely updates as needs evolve.

Qualifications

• Current Illinois RN licensure.
• Home Health experience preferred.
• CPR certification required.
• Valid IL driver’s license and auto insurance.
• HCHB EMR experience is a plus.
• Must possess excellent clinical nursing and customer service skills.

Responsibilities

• Complete a detailed initial assessment of the patient, family, and home environment to determine home health care needs.
• Monitor and regularly reevaluate the patient’s nursing needs.
• Initiate and revise the plan of care as patient status and needs change.
• Provide high-quality home health nursing and patient education.

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and skilled/custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Home Health Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

• Salary determined on experience and licensure

• Must possess current Illinois RN licensure
• Home Health experience preferred
• Current CPR
• Valid IL driver’s license and auto insurance policy
• HCHB EMR experience a plus
• Must possess excellent clinical nursing and customer service skills
