Job summary

Pathways Healthcare is seeking an On-Call Weekend Hospice Registered Nurse to provide home-based patient care in the Kingston, PA area. This part-time role covers Saturday and Sunday from 8:30 AM to 8:00 PM with base compensation plus visit rates.

Ideal candidates have 2+ years of nursing experience, a nursing degree, BLS CPR, and proficiency with EMR systems. A valid driver’s license and insured vehicle are required for in-home visits.

Qualifications

2+ years nursing experience; geriatrics preferred.
Nursing degree from an accredited program.
Proficient with electronic medical records (EMR).
Maintain current BLS CPR certification.
Valid driver's license and own a reliable, insured vehicle.

Responsibilities

Initiate appropriate preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by the physician.
Initiate and revise the plan of care as patient needs change.
Respond to after-hours calls during weekends (8:30 AM–8:00 PM).
Assess patient needs through triage and provide in-home care as needed.
Collaborate with physicians and the interdisciplinary team on the Plan of Care.
