Centric Home Health and Hospice is seeking a Registered Nurse to join our hospice team for weekend on-call coverage and after-hours patient visits. The role focuses on delivering compassionate, patient-centered care in home settings and coordinating with physicians and the care team.

Ideal candidates have RN licensure, CPR certification, and at least two years of clinical experience (hospice experience preferred).

• Current RN license.
• CPR certification current.
• Valid driver’s license with reliable transportation and auto insurance.

• Conduct initial and ongoing comprehensive patient assessments to determine hospice eligibility.
• Develop, implement, and revise individualized plans of care.
• Educate patients and families on disease processes, self-care, nutrition, and end-of-life needs.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Provide clinical direction to Hospice Aides and LPNs/LVNs.
• Maintain accurate, timely documentation of care and progress.
• Participate in on-call rotation and respond promptly to patient needs.

Overview

We are seeking a dedicated and experiencedRegistered Nurse (RN) to join our healthcare team as aHospice Weekend On-Call RN. The ideal candidate will possess a strong background in hospice and will be responsible for patient visits after hours and on weekends.

• Availability required: 5 pm on Friday through 8 am on Monday.

Key Responsibilities

• Conduct initial and ongoing comprehensive patient assessments ensuring hospice eligibility criteria is met
• Develop, implement, and revise individualized plans of care.
• Educate patients and families on disease processes, self-care, nutrition, and end‑of‑life needs.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Provide clinical direction to Hospice Aides and LPNs/LVNs.
• Maintain accurate, timely documentation of care and progress.
• Participate in on‑call rotation and respond to patient needs promptly.
• Follow infection control and safety procedures
• Uphold agency policies and professional standards

Minimum Requirements

• Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s).
• Current Registered Nurse (RN) state license.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication and interpersonal skills.
• Two years of clinical experience required; hospice experience preferred.

Work Environment

• Care provided in patient homes under varying conditions.
• Exposure to infectious diseases, blood, and bodily fluids possible
• Requires local travel and ability to work a flexible schedule.
• May involve prolonged standing, walking, and lifting up to 50 lbs.
