Job summary

Main Line Health is seeking a Weekend Inpatient Hospice Nurse Liaison/Case Manager to join our team. You will deliver coordinated, comprehensive nursing care to home-based hospice patients, using clinical judgment to address the physical, psychosocial, and spiritual needs of patients and families, thereby improving function and quality of life.

This role offers opportunities for involvement in Nursing Committees, continuing education, and a supportive team environment.

Qualifications

• RN license to practice in PA or a multi-state license via the Nursing Licensure Compact
• Valid Driver's license and automobile insurance
• Provider BLS (American Heart Assoc/American Red Cross) required
• CHPN preferred

Responsibilities

• Provide coordinated, comprehensive nursing care to patients in the home hospice setting.
• Apply critical thinking, analysis, and clinical judgment to meet physical, psychosocial, and spiritual needs of patients and families.
• Engage in hospital-wide Nursing Committees, Unit Councils, evidence-based projects, and precepting new staff.
