Job summary

Eden Health Hospice in Longview, WA is seeking a Weekend Licensed Practical Nurse (LPN) to join our 100% employee-owned team. Schedule: Every Saturday and Sunday, providing skilled nursing care to hospice patients under physician orders.

Qualifications

• Graduate of an accredited nursing program.
• Current LPN/LVN licensure in state of practice.
• Master's Degree in ministry, counseling, or social work preferred.
• CPR certification valid.
• Two years' full-time bereavement counseling experience within the last three years.
• Valid driver's license and reliable automobile.
• Pass criminal background check and national sex offender clearance.
• Experience as an LPN/LVN with recent acute care or home care preferred.

Responsibilities

• Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.
• Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines.
• Obtains necessary physician orders and collaborates with the physician as the patient's needs dictate.
• Assess and provide patient and family education and information pertinent to diagnosis and plan of care.
• Participates in coordination of hospice services, reporting identified needs for hospice interdisciplinary group members.
• Uses equipment and supplies effectively and efficiently.
• Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.

Education
