Job summary

Envision Hospice is seeking a compassionate Licensed Practical Nurse (LPN) for evening home visits across Kitsap County, WA. You will provide skilled nursing care, administer medications, document care, and educate patients and families on care plans.

Qualifications

• Current LPN license in Washington state.
• Active BLS certification.
• Hospice/home health or palliative care experience preferred.
• Strong clinical assessment and critical thinking.

Responsibilities

• Provide skilled nursing care to hospice patients per physician orders and policies.
• Administer medications, monitor conditions, and document care.
• Educate patients and families on care plans and comfort measures.
• Collaborate with RNs, social workers, and aides to support goals.
• Respond promptly to patient/family needs and urgent situations.
• Maintain compliance with state regulations and Medicare guidelines.
• Participate in weekend on-call rotation.
