Job summary

Envision Hospice is hiring a Licensed Practical Nurse (LPN) to provide direct hospice care in home settings, skilled nursing facilities, and other care environments in Washington state.

The role is full-time, days, based in King County with duties including medication administration, patient assessment, and family education, plus weekend on-call coverage. Competitive benefits and compensation reflect hospice experience.

Qualifications

• Current LPN license in the state of Washington.
• Active BLS certification.
• Hospice, home health, or palliative care experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage evening patient caseloads.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care to hospice patients per physician orders and policies.
• Administer medications, monitor patient conditions, and document care.
• Educate patients and families on care plans, symptom management, and comfort measures.
• Collaborate with interdisciplinary hospice team members to support patient care goals.
• Respond promptly to patient and family needs, including symptom management and urgent care situations.
• Maintain compliance with state regulations, company policies, and Medicare guidelines.
• Participate in weekend on-call rotation.
