Responsibilities

• Oversee case management guidelines for each team member.
• Assign patients to multidisciplinary team and adjust schedules as needed.
• Serve as first line for resolution of staff concerns and issues.
• Train, develop, and mentor staff; conduct orientations, patient assignments, and reviews.
• Communicate with support staff to assess needs and accountability.
• Collaborate with leadership on quality improvement and program effectiveness.
• Develop staff through training, joint visits, and coverage planning.
