Job summary
Residential Home Health and Hospice in Northbrook, IL, seeks a Hospice Registered Nurse Clinical Supervisor to lead the hospice clinical care team, coordinate care, and ensure quality standards in a home-based setting.
The role offers a base salary up to $105,000, plus quarterly bonus, comprehensive benefits, and a Monday–Friday schedule with rotating on-calls. Growth opportunities exist within a Top Workplace.
Qualifications
- Must hold a Registered Nurse license in the state of work.
- Minimum two years RN experience; hospice/home health preferred.
- Minimum one year nursing supervisor experience.
- BSN preferred.
- Excellent communication and organizational skills.
Responsibilities
- Manage the assigned hospice clinical care team and assist with care coordination.
- Provide guidance on interdisciplinary plans of care to ensure needs are met.
- Train, mentor, and evaluate hospice clinical care team on processes, protocols, compliance, and documentation requirements.
- Monitor clinical records to ensure visit, quality and satisfaction standards are met.
- Serve as liaison between field staff, referral sources, and hospice operations.
- Evaluate and address patient, caregiver, and customer concerns expeditiously.
- Monitor supervisory visits including home health aide and LPN visits.
- Analyze required supervisory reports using HCHB and our EMR.
- Attend all required interdisciplinary and staff meetings.
