Job summary

Lehigh Valley Health Network is seeking a Registered Nurse (RN) for Hospice Admissions on the weekend days. You will determine hospice eligibility, perform initial and follow-up assessments, and formulate an individualized plan of care with the physician and care team.

You will deliver patient and family education, coordinate with the interdisciplinary team, and provide end-of-life care with a patient-family centered approach.

Qualifications

• RN with either a Bachelor's or Associate degree in Nursing and hospice/home health experience.
• 2 years RN experience in acute care or hospice/home health required.
• Ability to travel within defined service area and work independently.

Responsibilities

• Performs comprehensive initial and follow up assessments of patient status including physical, psychological and environmental parameters.
• Collaborates with physician and hospice interdisciplinary team to determine needs and level of care.
• Delivers patient care per medical plan and protocols, with family education and goal attainment in mind.
• Provides direct nursing care with emphasis on end-of-life support and symptom management.
• Supports orientation of new staff and participates in service safety initiatives.
