Lehigh Valley Health Network, Inc. is seeking a Weekend Home Care Admissions Nurse (RN) to join our home care Pocono team.

This role focuses on determining appropriate levels of care, performing initial and follow-up assessments, and creating individualized care plans in partnership with the physician and interdisciplinary team. Responsibilities include delivering patient care per the medical plan, educating patients and families, and coordinating with hospital/clinic teams to ensure a smooth

Qualifications

• Specialized Diploma NLN approved/accredited nursing program.
• 1 year recent experience providing direct care in a healthcare setting.
• Knowledge of patient education techniques and principles.
• Ability to take initiative and function independently without direct supervision and as a member of the care team.
• Ability to travel to patient care assignments.
• Knowledge of Conditions of Participation.
• Knowledge of and skill in completing regulatory data set collection with a high level of accuracy.
• Ability to facilitate difficult and complex communication related to goals of care.

Responsibilities

• Performs comprehensive subjective and objective initial and follow up assessments of patient’s status with high accuracy.
• Delivers patient care based on physician plan of treatment and patient/family education.
• Collaborates with the physician and care team and relays significant changes in patient status.
