Job summary

Mission Healthcare in Ventura, CA is seeking a Hospice Admissions Registered Nurse (Weekend Shift) to initiate and revise the plan of care as patient needs evolve. You will determine nursing diagnoses from health assessments and create care plans that involve patients and families in the decision-making process.

You will educate patients and families, support activities of daily living, coordinate care as Case Manager when assigned, and collaborate with physicians and the interdisciplinary team

Qualifications

• Graduate from an accredited school of nursing. Bachelor’s degree preferred.
• Current state license as a Registered Nurse.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Minimum of 1 year of recent experience in home care, preferably in hospice.
• Valid driver’s license and reliable, insured transportation.

Responsibilities

• Initiates the plan of care and revises it as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and hospice actions.
• Provides health care instructions to the patient as appropriate.
• Assists the patient with activities of daily living and supports self-sufficiency and comfort care.
• Acts as Case Manager when assigned by Clinical Manager and coordinates patient care.
