VNAcare, a not-for-profit leader in hospice care, invites qualified RNs to join our Encino team as Admissions RN on a full-time evening shift. You will manage admissions, coordinate with physicians and hospitals, and help plan comprehensive Hospice services for patients in Southern California.

We offer competitive salaries, mileage reimbursement, a generous benefits package, and a sign-on bonus up to $5,000. CA license and acute-care experience preferred; EMR experience a plus.

Qualifikationen

• Maintains ongoing public relations and communication with physicians, hospital and facility staff, and referral sources regarding patient care and all available agency services.
• Evaluates potential Hospice patients and reports the findings of the Hospice evaluation to the patient's attending physician, and obtains orders for Hospice care if appropriate.
• Works cooperatively with hospitals, skilled nursing facilities, and medical groups to plan and coordinate Hospice services for referred patients; makes arrangements for supplies, equipment, or other needs for discharge.
• Completes the Hospice admission process and provides the team with information to ensure continuity of care.
• May provide nursing care to Hospice patients per standards of practice and scope of practice.

The Admissions RN position is open in our Encino location. This is a Full-time position during the evening shift from 12:00pm-8:30pm.

Requirements of the Admissions RN:

• Current RN license in State of CA
• Minimum one year of acute-care experience
• Experience in hospice preferred; EMR experience preferred

Admissions RN Duties Include:

• Maintains ongoing public relations and communication with physicians, hospital and facility staff, and referral sources regarding patient care and all available agency services.
• Evaluates potential Hospice patients and reports the findings of the Hospice evaluation to the patient's attending physician, and obtains orders for Hospice care if appropriate.
• Works cooperatively with the acute hospitals, skilled nursing facilities, and medical groups in the community to plan and coordinate Hospice services for referred patients. Makes arrangements for supplies, equipment, or other needs in preparation for discharge.
• Completes the Hospice admission process, including the required forms and documentation. Provides the Agency and members of the interdisciplinary team with all information to insure continuity of care.
• May provide nursing care to Hospice patients. Care is provided utilizing standards of practice and within scope of practice.
