Ambercare Hospice in Albuquerque, NM, is seeking a compassionate Weekend Intake Coordinator to join our team. This role serves as a vital connection point for patients, families, referral sources, and clinical teams during the hospice admission process and helps facilitate timely admissions.

You will collaborate with admissions, clinical, and operational staff, coordinate referrals, and ensure accurate documentation and timely communication.

Qualifications

• Age 18 or older.
• At least 1 year experience in homecare, hospice, and/or healthcare-related field preferred.
• Ability to complete documentation accurately, legibly, and timely.
• Ability to handle stressful and delicate situations.
• Interpersonal, verbal, and written communication skills required; valid driver's license and proof of insurance.

Responsibilities

• Receive referrals by phone or in person for designated department.
• Explain benefits to patients, families, and community contacts.
• Assist with finding alternative services when patient is not a candidate for hospice.
• Oversee scheduling of initial nursing assessment and admission.
• Maintain admission packets and ensure proper forms are available to admissions staff.
• Assist community outreach representatives with marketing activities to promote relations with referral sources.
• Complete admission paperwork as needed.
• Adhere to work schedule and maintain punctuality and attendance.
• Provide telephone and clerical support to staff as time allows.
• Maintain accurate clinical records and track progress of clients.
