This position requires flexibility to support weekend intake operations based on referral volume and business needs. The schedule includes:
Friday: 2:00 PM to 10:00 PM
Saturday: 12-hour shift
Sunday: 12-hour shift
Monday: Coverage through approximately 10:00 AM to 11:00 AM
Meaningful Work: Serve as a critical point of contact for patients, families, referral sources, and clinical teams, helping ensure timely access to hospice care and a seamless admission experience.
Collaborative Environment: Work closely with admissions, clinical, and operational teams to coordinate referrals, facilitate communication, and support patients and families throughout the intake process.
